A few questions before booking a rhinoplasty consultation:

1. Are you a previous patient of Quaba Plastic Surgery? Your name & address at that time, and your date of birth (so we can find your notes) 2. Have you had previous rhinoplasty or septoplasty surgery? Where? (NHS / private / abroad) and how long ago? 3. Have you had an injury to your nose that needed hospital treatment? 4. Have you ever had non-surgical rhinoplasty (HA fillers)? How long ago? 5. Is appearance your main concern?

If breathing through your nose is the main issue, we recommend an ENT referral first.

6. Are you otherwise fit and healthy?

I confirm I am 18 years of age or older. I consent to Waterfront Private Hospital storing these details to arrange my appointment.
